HealthFlex participants and their spouses are encouraged to complete the Health Check questionnaire and Blueprint for Wellness by August 31, 2024.

To avoid a higher deductible in 2025, both participants and their covered spouses must complete the Health Check questionnaire. While all covered West Ohio Clergy completed the Health Check questionnaire, fewer than half of their spouses have done so. The questionnaire takes 10 – 20 minutes to complete. You got this!  Additionally, many have not completed their annual Blueprint for Wellness exam. Remember, the deadline to complete this exam and receive the $100 reward is also by August 31st. Completing it after this date means missing out on the incentive.

You can also call 1-855-623-9355 to register. Again, you will earn $100 for completing the Blueprint for wellness exam by August 31, 2024.
